4-H Achievement Applications

🌟 It’s 4‑H Achievement Application Time! 🌟

It’s that time of year again when 4‑H’ers are encouraged to reflect on and record their achievements and learning experiences from the past 4‑H year so their accomplishments can be recognized.

COUNTY AWARDS
4‑H Achievement Applications are available for all age divisions — Junior, Intermediate, and Senior. County-level applications will be used to award project medallions and pins within each division.

STATE AWARDS
At the state level, the 4‑H Achievement Application serves as the standard application form for senior 4‑H’ers (ages 15–18). These applications are used to select:

  • State award winners
  • Nebraska delegates to National 4‑H Congress
  • Nebraska 4‑H Foundation scholarship recipients

This is a wonderful opportunity for 4‑H members to highlight their growth, achievements, and dedication throughout the year!

Achievement Applications 

AWARD AREAS (For Both Junior and Senior)

  • Agriculture (Animal Science, Veterinary Science, Plant Science)
  • Leadership and Entrepreneurship (Community Action, Community Development, Communication & Expressive Arts, Civic Education & Engagement, Leadership, Entrepreneurship)
  • Healthy Living and College and Career Preparation for My Future (Health, Nutrition, & Fitness, Social & Emotional Well Being, Consumer & Family Science, Career and College Readiness)
  • STEM (Environmental Science, Engineering & Technology)
Junior Achievement Application for ages 8-14 (county-only)
Senior Achievement Application for ages 15-18 (state-eligible) 
A complete Achievement Application includes:
  1. Achievement Application - all areas complete (see examples above)
  2. Limit “My 4‑H Experiences” section to a maximum of three (3) pages for juniors and four (4) pages, single-spaced, 12-point font. A fifth page is allowed for delegate applicants.
  3. In the selection process, significant accomplishments in your 4‑H experience, leadership, community involvement, and career spark are given more consideration than the number of 4‑H projects completed.
  4. Creativity is encouraged, use bullet lists, photos, infographics, etc. to help tell your 4‑H story.

COMPLETING THE APPLICATION

  • Limit “My 4‑H Experiences” section to a maximum of three (3) pages for juniors and four (4) pages, single-spaced, 12-point font. A fifth page is allowed for delegate applicants.
  • In the selection process, significant accomplishments in your 4‑H experience, leadership, community involvement, and career spark are given more consideration than the number of 4‑H projects completed.

    1. Copy of current transcripts
    2. ACT/SAT scores

County 4‑H Achievement Applications are due on the second Friday in November of each year and will be submitted electronically by attaching the document to an email. polk-county@unl.edu.
