Required Identification

Beef - Market and Breeding Combined Form - Adams Beef-Market and Breeding Affidavit 2025: County Fair - 4-H Ear Tag, EID or Tattoo. State Fair EID or Tattoo plus DNA sample & online nomination.

Beef – 1st Year Bucket Calf –  Adams 1st Year Bucket Calf Revised 2025: County Fair - 4-H Ear Tag (County only) Calf must be born between Jan. 1st and April 15th,  Ear tag picked up by April 15th

Beef – 2nd Year Bucket Calf – Adams 2nd Year Bucket Calf Revised 2025 (marked Market or Breeding): County Fair - 4-H Ear Tag (County only)

Beef – Stocker Feeder Calf – Adams Feeder Calf  Affidavit Revised 2025: County Fair - 4-H Ear Tag or EID. State Fair – EID, DNA sample & online nomination.

Beef – Cow and Calf Form Adams County Cow Calf-Affidavit 2025: County Fair - 4-H Ear Tag, EID or Tattoo (both Cow and Calf ID’d on form).

Cats – Cat-Ferret-Vaccination-Certification Form (NE SF262) (rev. 5/17) with current Rabies vaccination (County Only)

Dairy Cattle – NE 4-H/FFA Dairy Cattle (rev. 1/16)

Dairy Goats – NE 4-HF109 (rev. 1/09)

Dogs – Adams NE Dog Vaccination form with current Rabies & Bordetella vaccinations (NE SF263) (See Fair book Animal Health Rules #8)

Ferrets – Cat-Ferret-Vaccination-Certification Form (NE SF262) (rev. 5/17) with current Rabies vaccination (County Only)

Meat Goats - Market and Breeding Combined Form - Adams Meat Goat Market and Breeding Affidavit 2025: County Fair - 4-H Ear Tag plus Scrapie Tag. State Fair – Scrapie Tag, DNA Sample & online nomination.

Goats – Pygmy – Adams Pygmy Goat Affidavit Revised 2025 (with Pictures or Drawing)  (County Only)

Horse – Horse ID Certificate NE F29777 (rev. 2017) – with Pictures/Drawing for County Fair and State Fair by respective deadlines

Poultry – Market Broilers: County Fair - Must be purchased thru the Adams County Extension Office. 

Rabbits – County Fair – None needed. State Fair - Tattoo (via online entry only) (County Verification for State - Adams Rabbit Affidavit revised 2025)

Sheep - Market and Breeding Combined Form - Adams Sheep Market and Breeding Affidavit 2025: County Fair - 4-H Ear Tag plus Scrapie Tag. State Fair – Scrapie Tag, DNA Sample & online nomination.

Swine – Adams Market Swine Affidavit Revised 2025: County Fair - 4-H Ear Tag or EID (NO Notches). State Fair – (for Breeding Swine also) EID, DNA Sample & online nomination 

Breed Registration Papers – Copy of the breed registration papers turned in with corresponding Identification form for any Market or Breeding animal that is a possible animal to be split out into a special “Breed Class” at County Fair is highly recommended or you can bring them to Fair on Animal Check-in to show the Superintendent.  The registration papers must show the exhibitor’s name or a co-owner who is an immediate member of the exhibitor’s family.  New in 2025 - Family Farm or Ranch Names will not be accepted.

Premise ID and Scrapies Tag

PREMISE ID

Adams County and State 4-H needs one on file for you to show Beef, Sheep, Goats, Swine, Rabbits, Poultry at the Fairs.

A premises identification number (PIN) is a unique code that is permanently assigned to a single physical location.  A premise ID is administered by each State and allows animal health officials to quickly and precisely identify where animals are located in the event of an animal health or food safety emergency.

If you do not know the Premise ID take a couple of minutes to call the Nebraska Department of Agriculture at 402-471-2351 or you can email them at agr.cvine@nebraska.gov to obtain the Premise ID (no cost)

Scrapie Tags

Market Sheep, Breeding Sheep, Market Goats, Breeding Goats need USDA Scrapies tags for identification for County Fair and State Fair. 

Scrapie Tags will serve as the official 4‑H identification for Goats & Sheep at State Fair and the secondary ID for Adams County Fair. 

Scrapie Tags ARE NOT provided by the Extension Office, you as the family must request them.

2026 Affidavit Forms have been newly created or revised this year.

These Identification forms are to be turned in to the Adams County Extension Office by appropriate deadline dates.

We use them to Identify the eligible animals that can be shown at Adams County Fair.  

Horse ID sheets and Dog Vaccination Forms are also used for State 4-H Events / Fair
