The 4-H record book tells others your story. Don’t miss the opportunity to tell YOUR 4-H story!
All 4-H End of the Year Reports and 4-H Awards Books are due to the Extension Office by September 30.
👉 Clover Kids do not submit 4-H record books. Every enrolled Clover Kid will receive a special recognition at their county’s fall banquet.
Many awards are available for youth ages 8–18!
The local 4-H Council supports award pins to recognize outstanding learning and performance in several project areas. Some award pin areas include specific livestock species, specific static area and shooting sports. For a complete list go to the Extension Office.
- Youth may apply for up to three pins per year.
- Must be in the project for two or more years.
- May win once as a Junior and once as a Senior.

- Fill out the County Awards Application Form
- ⚠️ If the form is not in the front, your record book will not be considered!
- Place it in the front of your 4-H Yearbook
✍️ Record Book Options for Juniors (ages 8–11)
Option 1: Yearly Story Format

- Yearly Story: a general story about your 4-H year.
- Project Stories: one page or less for each project area you’re applying in.
- Supporting Materials/Pictures: up to but not over 4 pages.
- Return to Extension Office by September 30.

- Complete the Achievement Application (reflective of your age)
- Part 1: Describe your overall 4-H experiences since joining.
- Part 2: Describe your 4-H Experiences from the past year.
- Leadership Experiences
- 4-H Leadership Experiences
- Community Involvement/Service-Learning
- Describe two times you volunteered or served your community (through 4-H or other)
- Career Spark
- Share how your 4-H Experiences have inspired or connected to your future career interests.
- Return to Extension Office by September 30.
- Non-4-H Leadership Experiences

- Return to Extension Office by September 30.
- Fully complete the Nebraska State 4-H Achievement Application (reflective of your age)
- State applications are separate from county applications and do not come to your local county Extension Office.
- County awards do not require school transcripts.
