🌟 It’s 4-H Achievement Application Time! 🌟
It’s that time of year again when 4-H’ers are encouraged to reflect on and record their achievements and learning experiences from the past 4-H year so their accomplishments can be recognized.
COUNTY AWARDS
4-H Achievement Applications are available for all age divisions — Junior, Intermediate, and Senior. County-level applications will be used to award project medallions and pins within each division.
STATE AWARDS
At the state level, the 4-H Achievement Application serves as the standard application form for senior 4-H’ers (ages 15–18). These applications are used to select:
- State award winners
- Nebraska delegates to National 4-H Congress
- Nebraska 4-H Foundation scholarship recipients
This is a wonderful opportunity for 4-H members to highlight their growth, achievements, and dedication throughout the year!
Clover Kid Record Book
Achievement Applications
- Form 0-11 ( You are allowed to pick three award areas to apply for. You will only be allowed to win the county award once per age division. please be sure to include 4-H Leader Signature)
- Form 0-11 must be included with all applications except Clover Kid Record Books.
Junior Achievement Application for ages 8-14 (Jr. 8-11, Int. 12-14) (county-only)
Senior Achievement Application for ages 15-18 (state-eligible)

A complete Achievement Application includes:
- Form 0-11 (completed with signatures, award areas circled, name)
- Achievement Application - all areas complete (see examples above)
County 4-H Achievement Applications are due on the second Friday in November of each year and will be submitted electronically by attaching the document to an email. Send to cuming-county@unl.edu.
