Waivers: please sign all waivers that apply to student including overall waiver, shooting sports, & horse waivers. Emergency contact: this needs to be someone other than mom or dad. We will always try to contact mom & dad 1st. the emergency contact is whom we can call if we cannot get ahold of either parent/s. (aunt, uncle, grandparent/s or someone close to family). Second parent info needed: in 4-H online we need second parent & their phone numbers listed. If they have a different address that needs filled in also. If they want an email copy of correspondence, add the email and check the box.
